You can make this pretty easily out of scrap plastic. What, you don't have scraps of plastic scattered around your house? Go to your local hardware store and find a no parking sign. Cheap and easy! Cut one piece of plastic to the size of the pocket in your sleeve. Cut another piece to the size of the window in the sleve pocket. Glue the smaller part onto the larger, such that when you stick it in the wrist pocket, the smaller piece is centered in the opening. Cut smaller pieces of plastic to make the buttons, then paint it all with your favorite paint. I used white plastic spray paint as the base, then Testor's model enamel for the buttons
